Preparing For Your Visit To Discuss Your Child's Behavior
1. Who should attend this?
- * The child and parent(s). NO SIBLINGS
- * If more than 2 parents are going to attend this visit, such as step-parents or grandparents, please let scheduler know this and make a 90 minute appt, not the usual 60 minutes.
2. What do I need to know about this appointment?
- * This first appointment will be with Michele Muskus, ARNP, our nurse practitioner, specially trained in pediatric and adolescent behavioral problems.
- * Ms. Muskus will conduct a detailed history of the child and parents.
- * No medication will be prescribed at the initial visit
3. What should be brought to this visit?
- * Completed Vanderbilt and SNAP forms found on our web site and downloadable. These forms are to be completed by all relevant parents and all teachers.
- * Some familiar toys that your child would enjoy playing with during the visit.
- * Any and ALL evaluations done by other medical, psychological, or school sources.
- * Reports cards or school work which you feel would contribute to this assessment
4. What should I do if I have more questions before or after this visit?

24 HOUR CANCELLATION IS REQUIRED OR A $50 NO SHOW CHARGE WILL BE APPLIED TO YOUR ACCOUNT.
A large block of Ms. Muskus' time is set aside for this kind of appointment and without adequate notice no other appointments can be made to fill this time.
We recognize things come up and appointments have to be rearranged.
